Photo Release Form

I have sole rights to the photograph submitted by me for consideration in the Green Faces of LUSH contest. I grant permission to LUSH Fresh Handmade Cosmetics to use this photograph provided by me on this date, through this website, for use in LUSH Fresh Handmade Cosmetics publications such as catalogs, newsletters, brochures, advertisements, and electronic versions of the same publications or on LUSH Fresh Handmade Cosmetics websites or other electronic forms or media, and to offer them for use or distribution in other publications, electronic or otherwise, without notifying me.

I hereby waive any right to inspect or approve the finished photographs or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photograph.

I hereby agree to release, defend, and hold harmless LUSH Fresh Handmade Cosmetics and its agents or employees, including from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction or production of the finished product, its publication or distribution.

I am 18 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release.

Contest Terms and Conditions

LUSH reserves the right to reject any application if we determine, at our sole discretion, and without notification, that the submitted material is incomplete or unsuitable for the contest. LUSH reserves the right to edit or modify any application for spelling, grammar, readability or length at our sole discretion with no prior notification to entrant. LUSH will hold and review all entries for up to two business days prior to posting after the April 7th initial launch of the voting phase of the contest.
